Hospital appointments or emergency doctor, dentist or opticians. Please provide the office with a copy of the hospital letter etc. This is the parent's responsibility and in some cases, without the letter, the session will be marked as unauthorised

This form is to be used for exceptional circumstances. Examples include: funerals, transition to secondary school visits, embassy appointments.

Please note, that term time holidays will not be authorised.

Non-urgent/routine appointments should always be made outside of school hours when possible.

Not all requests are authorised by the school.
